About this role: 

As a Sleep Medicine Physician, you will work with a team of healthcare professionals committed to providing quality care and excellent patient experience.?

• Sleep medicine physician needed to join our sleep medicine team at American Fork Clinic in American Fork, Utah
• Sleep lab located in the clinic at American Fork
• Outpatient only with no call responsibilities
• Mid-level provider assistance
• Employment position with the Intermountain Medical Group
• A 501c3 company
• This position includes a?$15,000?sign-on bonus and up to $20,000?in relocation assistance

What you’ll bring: 

• Sleep medicine fellowship required
• Board Certified/ Board Eligible in sleep
• Unrestricted Utah MD or DO license or the ability to obtain an unrestricted license
• U.S. ACGME Residency
• BLS certification
• Ability to successfully complete Intermountain Health’s credentialing process 
• Candidates requiring Visa sponsorship will be reviewed on an individual basis
