The VA Northern California Health Care System (NCHCS) is currently recruiting a full-time, board-certified Geriatric Psychiatrist who is value-driven, recovery- and diversity-oriented, and possesses energy, compassion, and a pursuit of excellence as a consultant for the Community Living Center (30%) and Home-Based Primary Care (70%) programs, located at the Martinez VA Medical Center and Pleasant Hill, respectively. Psychiatrists who will be completing a Geriatric Psychiatry fellowship and intending to complete board-certification are also encouraged to apply.
Academic appointments with VA affiliates are encouraged. This position does not have an on-call feature.
The Home-Based Primary Care (HBPC) is an interdisciplinary team of Physicians, Nurse Practitioners, Psychologists, Pharmacists, Nurse Case Managers, Dietitians, Social Workers, Respiratory Therapists, and Occupational Therapists that provides primary and palliative care in the home to medically complex, usually home-bound geriatric Veterans. It is a nationally recognized program that reduces hospitalizations and institutionalization of Veterans and has been replicated by other health care systems.
The Community Living Center/Center for Rehabilitation and Extended Care (CLC/CREC) is located at 150 MUIR ROAD, MARTINEZ, CA 94553 and provides multiple in-patient services including:
This position involves some onsite care for HBPC and for CLC in the event of an urgent issue. Much of the work is done virtually. Mental Health support for HBPC and CLC is critical to ensuring positive outcomes of care. The most common conditions seen include dementias with difficult behaviors, depression, anxiety, and psychoses. Administrative time for supervision of HBPC Psychologists is provided.

The Department of Veteran Affairs Northern California Health Care System (VANCHCS) is an integrated health care delivery system, offering a comprehensive array of medical, surgical, rehabilitative, mental health and extended care to veterans in Northern California. The Martinez VA Community Based Outpatient Clinic (CBOC) offers a full range of medical, surgical, mental health, and diagnostic outpatient services, including nuclear medicine, ultra-sound, CT and MRI. The clinic also operates a freestanding ambulatory surgery center. Additional services available at this facility include an infusion clinic, substance abuse treatment, pain management, rehabilitation medicine, phototherapy, audiology and speech pathology, women's health, Home Based Primary Care, EMG, EEG, movement disorder and a vascular laboratory. Also situated on this campus is a Community Living Center (CLC), a 120-bed extended care facility, offering subacute, rehabilitation, restorative, palliative, respite and transitional care. In addition, the CLC operates a neurocognitive unit, subacute unit, and a 23-hour bed program in support of post-procedural ambulatory care services.
