Madison, TN – a NE suburb of Nashville
Smyrna, TN – 30 minutes SE of Nashville

This fast-growing, senior-focused, team approach to primary care provides a positive and well-balanced environment. This model is positioned to provide better outcomes for senior patients as we provide multi-disciplinary services and coordinated care supported by analytics and community relationships.

• Mon - Friday schedule
• Average 10-12 patients per day
• Time with patients - hour-long initial and typically 45 minute-long appointments

• Guaranteed base salary plus up to 20% bonus

Requirements:

• Board Certified or Board Eligible Internal Medicine, Family Medicine or Geriatrician
• Active and unrestricted DEA
• Knowledge of Medicare guidelines helpful
• Knowledge of HEDIS quality indicators
