Per Diem Family Medicine Physician Heater Road Clinic - Lebanon, NH Dartmouth Health system is currently recruiting a BC/BE per diem Family physician to join our academic Primary Care practice at our Heater Road Clinic in Lebanon, NH located in a beautiful new outpatient facility three miles from Dartmouth-Hitchcock Medical Center. The Family Medicine team at Heater Road is an integral part of a dynamic Primary Care group which has a focus on clinical and educational quality improvement, population health, and development of new models of care delivery.  We strive to provide team based, high-value quality care to all patients within an academic setting where teaching, learning and scholarship is central to our work and mission. Unique aspects of our practice include Family Medicine physicians who have additional training in Sports Medicine, Sleep Medicine, Functional Medicine, Addiction Medicine, and we provide the potential for leadership development opportunities. Our integration within Dartmouth-Hitchcock Medical Center provides for unique collaborative relationships across an extensive array of specialties, as well as tremendous technical and administrative support. Our community has a diverse patient population, and we have a robust team set up to work with patients struggling with the various Social Determinants of Health. The Department of Community and Family Medicine offers teaching, research and career development opportunities to all of our physicians.  The ideal candidate would be a skilled clinician, an enthusiastic teacher and scholar committed to improving the health of our patients through high quality team-based care.
