Family Medicine Physician Alice Peck Day Family Practice - Lebanon, NH Dartmouth Health System is currently recruiting a Family Practice physician to join our Primary Care practice at our Multi-Specialty Clinic in Lebanon, NH located in a beautiful new outpatient facility 5 miles from Dartmouth Hitchcock Medical Center. The Family Medicine team at Alice Peck Day Primary Care is an integral part of a dynamic group that has a focus on clinical quality improvement, population health, and the development of new models of care delivery.  We strive to provide team-based, high-value quality care to all patients. Our integration within Dartmouth Health System provides for unique collaborative relationships across an extensive array of specialties, as well as tremendous technical and administrative support. Our community has a diverse patient population, and we have a robust team set up to work with patients struggling with the various Social Determinants of Health.
