Highlights

Community Physician Network is seeking a fellowship trained Sleep Medicine physician to be based in Kokomo and eventually Westfield.

·         Opportunity to work with experienced, AASM accredited providers

·         No call & outpatient only practice

·         Monday through Friday; 8am – 5pm with a mix of virtual & in-person visits

·         Epic EMR

Licensure & Certification

·         Board Certified in Sleep Medicine

·         Eligible for medical licensure in Indiana
