Position Summary:

Join our dynamic care team, which includes Physicians, Advanced Practice Professionals, Nurses, Social Workers, Case Managers, Care Coordinators, Medical Assistants, Behavioral Health Support, quality-focused Coders, and Referral Coordinators. This collaborative environment empowers our primary care physicians to manage a smaller patient load, allowing them to dedicate more time to delivering exceptional care.

Primary Responsibilities:

  • Evaluate and treat patients in accordance with established standards of care.
  • Coordinate patient services, including specialty referrals, hospital and skilled nursing facility (SNF) collaboration, durable medical equipment, and home health care.
  • Actively participate in daily huddles as a key source of medical expertise within the care team.
  • Collaborate with the Lead Physician and Center Administrator to foster a cooperative practice environment through a professional and approachable demeanor.
  • Ensure timely and comprehensive medical record documentation, working alongside a quality-based coder for optimization.
  • Adhere to policies and protocols established by Clinical Leadership.
  • Regularly meet with the Lead Physician to discuss quality of care, review outcome data, and address medical records concerns.
  • Focus on direct patient care, balancing patient-facing time with essential administrative duties related to patient care.

Required Qualifications:

  • Graduate from an accredited MD or DO program.
  • Current, unrestricted medical license or willingness to obtain one in Arkansas, with eligibility to secure licenses in other states as needed.
  • Board Certified or eligible for certification (ABMS or AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine is preferred.
  • Active and unrestricted DEA license.
  • Medicare and Medicaid Provider Numbers.
  • Strong interpersonal and communication skills for effective collaboration with colleagues and patients.
  • Commitment to a team-based care model.
  • Adaptability to practice within a value-based care setting.
  • Exceptional patient and customer service skills.
  • Proficient in basic computer skills, including email and electronic medical records (EMR).

Preferred Qualifications:

  • Familiarity with HEDIS/STARS metrics.
  • Knowledge of CAHPS and HOS surveys.
  • Experience in medically underserved or culturally diverse communities.
  • Proficient in using electronic health records.
  • Background in Medicare Advantage Global Risk Models.
