Position Summary:

Our care team includes Physicians, Advanced Practice Professionals, Nurses, Social Workers, Case Managers, Care Coordinators, Medical Assistants, Behavioral Health Support, Quality-Based Coders, Referral Coordinators, and more. Primary Responsibilities:
- Provide patient-centered care in an outpatient setting, adhering to established standards of care.
- Coordinate patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care as needed.
- Actively participate in daily huddles with the care team, sharing medical insights and collaborating on patient management.
- Support the Lead Physician and Center Administrator by fostering a collaborative and professional team atmosphere.
- Complete daily medical documentation with support from quality-based coders to ensure thorough, accurate records.
- Adhere to policies and protocols set by Clinical Leadership.
- Regularly review care quality, outcomes, and medical records with the Lead Physician.
- Balance clinical responsibilities, including patient-facing time and administrative duties, all focused on direct patient care.
Required Qualifications:
- MD or DO degree from an accredited medical program.
- Current, unrestricted medical license or ability to obtain a license in the practicing state; willingness to gain licenses in additional states as needed.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine (ABMS or AOA).
- Active, unrestricted DEA license.
- Medicare Provider Number.
- Medicaid Provider Number.
- Strong interpersonal and communication skills with colleagues and patients.
- Commitment to team-based care.
- Adaptability and commitment to value-based care principles.
- Excellent patient service skills.
- Basic computer skills, including proficiency with email and EMR systems.
Preferred Qualifications:
- Familiarity with HEDIS/STARS measures.
- Understanding of CAHPS and HOS.
- Experience serving medically underserved or culturally diverse populations.
- Proficiency with electronic health records.
- Experience in Medicare Advantage Global Risk Models.
