Overview of the Role:
The Extensivist is a licensed Physician who provides services to the senior patient population across multiple settings. S/he will see, treat, and manage patients primarily in the skilled nursing facility setting and in patient’s residences. S/he will also see patients occasionally in the hospital for patient engagement.
