Responsibilities

We are seeking to recruit a full time, practicing Medical Director of Population Health to serve as a part-time practicing primary care physician at Ascension Medical Group and as a part time Medical Director for the Ascension Seton Health Alliance. We expect the position to be 50% clinical, 50% administrative but could change as the program and staffing grows.This role is a hybrid role which requires clinic time, but also allows for flexibility for virtual/remote work through the Ascension Seton Health Alliance. 

 

The Medical Director of Population Health is a key leader for the advancement of value-based care at Ascension Texas. The Medical Director is responsible for helping to create and implement the vision for population health, to design or identify the programs, to facilitate the clinical operations, and to analyze the results of population health department activities and overall value-based care performance for patient populations. This role will provide clinical oversight for all value-based programs managed by the Ascension Seton Health Alliance which is a clinically integrated network of over 4,000 physicians and advanced practice providers managing over 230,000 value-based lives.

The ideal candidate will have 10+ years of experience in practicing value-based care within a primary care setting. Additionally, the candidate should possess the leadership competencies required to help drive organizational change to successfully promote the expansion of value-based care across the ministry. Responsibilities Include:

  • Understand and apply the principles of Population Health.
  •  Provide medical leadership for Population Health within Ascension Medical Group and aligned independent practices within the Clinically Integrated Network (CIN).
  •  Collaborate with System leadership to guide pursuit of the “Triple Aim” of healthcare consistent with Ascension’s strategic goals.
  • Communicate the principles of Population Health and provide education to employed and independent physicians in the CIN.
  • Collaborate with the Clinical Integration Committee to achieve superior quality of medical care defined by evidence based principles of care.
  • Collaborate with the CIN leadership team to develop and interpret performance measures for a population of patients.
  • Provide medical leadership and partner with the Vice President of Population Health and Director of Population Health to lead and manage care management, quality and overall performance for population health.
  • Collaborate with the Managed Care Department for the purpose of providing appropriate assistance in managed care contracting from the perspective of value based contract performance.
  • Partner with the Vice President of Population Health to lead organizational efforts to be successful in transition from volume based care to value based care.
  •  Accountability including quality and financial performance of all Value Based Contracts for the Ascension Seton Health Alliance in partnership with the Director of Population Health.
  • Accountability including communication and education of stakeholders regarding Population Health related quality, utilization and cost measures.
  • Clinical leadership for ACO development and operations in areas of Population Health, including Ascension Medical Group employed physicians as well as independent affiliated physicians.
  • Serving as a practicing primary care physician at a designated Ascension Medical Group primary care clinic.

Requirements

Licensure / Certification / Registration:

  • One or more of the following required:
    • Licensed Physician MD/DO credentialed from the Texas Medical Board.
    • Podiatrist credentialed from the Texas Department of Licensing & Registration (TDLR).

Education:

  • Doctor of Medicine (MD) or Doctor of Osteopathy (DO) or Doctor of Dental Surgery (DDS) or Doctor of Dental Medicine (DMD) or Doctor of Optometry (OD) or Doctor of Podiatric Medicine (DPM) required.
