Effingham location. Procedures done in ambulatory surgery center in town. Pain injections and implants, no pain med management.

Highlights include:

• $650,000 + bonus based on RVU production
• $75,000 sign-on bonus
• $60,000 total retention bonus (paid $20,000 at the end of years 1, 2 & 3)
• Quality bonus up to $50,000 annually (eligible upon earning base salary)
• 33 days of time off
• 5 days + $6,000 CME annually
