Franciscan Alliance Inc. seeks Physician at its facility located at 3920 St Francis Way, Ste 209, Lafayette, IN 47905. Responsible for rendering designated on-site and off-site professional and competent health care services and medical care to patients of FPN. Prepare specific, accurate and timely medical care to patients of FPN. Prepare specific, accurate and timely medical records, using FPN's EMR technology. Supervise and provide medical direction to persons who assist Physicians in rendering professional services at FPN. Assist in educating and training employees and office staff recruited to support at FPN. Assist in educating and training employees and office staff recruited to support Physician in the delivery of services. Participate in FPN and Franciscan Alliance quality and utilization review mechanisms designed to improve the quality and efficiency of the physician services, including, but not limited to, mechanisms to address utilization review procedures requested by any current or future managed care payers or other external parties. Cooperate in FPN and Franciscan Alliance efforts to reduce variation from best practices and to enhance service, patient satisfaction, efficiency of care and efficiency of resource consumption. Meet performance and quality criteria and cooperate with the Centers for Medicare and Medicaid Services ("CMS") initiatives as designated by FPN and Franciscan Alliance form time to time which may include, without limitation, one or more of the following: claims based measures, a Consumer Assessment of Health Providers and Systems ("CAHPS") survey, data from Electronic Health Record Incentive Programs, the Centers for Disease Control and Prevention National and Healthcare Safety Network, Hospital Compare, and those reported by each Medicare ACO through the Group Practice Reporting Option ("GPRO"), and any other performance measures adopted by CMS from time to time. All such initiatives shall be developed, adopted and implemented in accordance with FPN and Franciscan Alliance policies. He will cooperate in measurement and evaluation of current practices and evaluation of improved practice performance. Assist FPN in ensuring that the provisions of medical services comply with all applicable policies and requirements of licensing, regulatory and accrediting bodies. Assist FPN as requested with third-party credentialing, billing, and collection matters in a specific, timely and accurate manner, including Medicare and Medicaid, and private third-party payors and will coordinate medical care and maintain effective professional relationships with FPN physicians, as well as those on Hospital's Medical Staff. Will treat patients aged newborn to 99. Will complete well exams and physicals on adults 18 years old and up including ordering preventative care such as vaccinations and screening testing. Will see patients who have an acute problem, such as ear pain, illness or flu symptoms. Will complete Annual Medicare Visits on patients over the age of 65. Will take care of patients as a follow up to Emergency room visits. Will care for patients who are discharged from the hospital within 10 days of their release. Will see new patients who want to establish care with him, which includes an exam, history and medication review. Will take care of well child visits- ages newborn to 18. Will complete the needed sports physicals for adolescents, ages 12-18. Will take care of all ages of patients who need care for their chronic conditions, such as hypertension, diabetes and osteoarthritis. Will order labs and use the data to adjust medication doses for chronic or acute conditions such as hypertension, depression and neuropathies.

REQS: The position requires an MD or DO degree. The employer will accept ECFMG equivalency to MD. Additionally, the applicant must have: (1) License to practice medicine in Indiana; (2) Completed a residency in Family Medicine from an accredited institution; and (3) Board Certified or Board Eligible in Family Medicine.
