Outpatient Palliative Care Physician - Wall, New Jersey

Hackensack Meridian Health (HMH) is seeking a BC/BE palliative care physician to fill the role of Outpatient Palliative Care Physician. The successful candidate will play a crucial role in enhancing the quality of palliative care across Hackensack Meridian Health, in both home and outpatient settings.

 

RESPONSIBILITIES 

  • Provide direct palliative care consultations in various settings, dedicating focused time to patient care.
  • Ensure the standardization of processes and protocols, actively contributing to a network-wide palliative care harmonization workgroup.
  • Collaborate with advanced practice and registered nurses, social workers, and administrative team members to coordinate medical care with community physicians.
  • Contribute to the expansion of a collaborative program, demonstrating a commitment to clinical excellence and innovation.
  • Partner with physician leaders to develop education plans, create curricula, and present on topics such as serious illness communication and advanced illness management.
  • Contribute to the establishment of a palliative care fellowship program.

OPPORTUNITY REQUIREMENTS: 

  • MD/DO, BC/BE in palliative medicine
  • Ability to assess, evaluate, plan, and implement care in complex and life limiting situations
  • Capable of offering specialized knowledge and skill to all members of the health care staff
  • Team oriented and able to provide personalized patient care as well as family support and demonstrate clinical outcomes
