How You Will Help Our Patients

  • Through the honor of being welcomed into patients’ homes, you’ll be a home-based primary care physician who will:
    • Assess patients’ full spectrum of needs at least annually, including medical psychological, social, financial, and environmental;
    • Understand patients’ values and goals and thoughtfully incorporate them into each care plan;
    • Guide patients with multiple chronic medical conditions and their caregivers through complex medical decision making;
    • Proactively manage patients’ health through preventive care as dictated by geriatric medicine guidelines as appropriate for patients age, prognosis, and medical complexity;
    • Manage both acute and chronic disease states;
    • Guide management of transitions of care;
    • Actively quarterback patients’ care through proactive conversations with other physicians, providers, and caregivers to ensure alignment of treatment plan.
    • Facilitate advanced care planning conversations and provide palliative care support to patients as appropriate;
    • Build deep and strong relationships with patients and caregivers, to help them achieve the health outcomes that are most important to each person;
  • You’ll act as the day-to-day clinical leader of your multi-disciplinary team, supporting team members in providing holistic care to our patients.
  • You’ll collaborate with our staff to support the delivery of needed medical services, medications, and equipment.
  • You’ll regularly provide after-hours medical coverage for patients (as part of your scheduled on-call duties).
  • You’ll document encounters in a timely manner in Welcome Health’s EMR.
  • You’ll provide same-day assessments and treatment plans for patients with acute needs.
  • You provide healthcare through telehealth visits as appropriate.
  • You’ll participate in patient care and management meetings including (but not limited to) daily huddles, weekly interdisciplinary team meetings, monthly clinical staff meetings, and others to support Welcome Health’s commitment to outstanding clinical care.
  • You’ll implement initiatives to help to ensure industry-leading standards of care, and to help Welcome Health achieve industry-best quality metrics (HEDIS, CAPHS, etc.).
  • You’ll stay abreast of medical research and trends, through continuing education courses, professional meetings and journal reviews (all with Welcome Health’s support).
  • You’ll bring proficiency in routine physical exams, as well as skills needed for in-home interventions, including blood draws, wound care and dressing changes, IV and catheter insertions and management.
  • You’ll take great pride in your patient satisfaction levels.
  • You’ll arrange for transfers and placements to appropriate levels of care.
  • You’ll perform other duties as needed.

 

What You’ll Need to Succeed as a Welcome Health PCP
To perform this job well, you’ll of course need to be a great teammate and problem solver, bringing your best technical and interpersonal skills to bear. But you’ll also need the following:

  • Medical Doctorate or Doctorate of Osteopathy
  • Board Certification in Internal Medicine, Geriatrics or Family Practice (or be Board-eligible for up to one year if you’re a new grad).
  • A current, valid, non-sanctioned, non-restricted license to practice medicine in California.
  • The ability to meet Welcome Health’s credentialing requirements.
  • Familiarity with Medicare guidelines.
  • Familiarity with CPT and ICD-9 codes.
  • A malpractice profile that meets Welcome Health’s risk guidelines.
  • A deep knowledge of clinical standards of care.
  • Helpful but not required: an awareness of UM standards, NCQA requirements, CMS guidelines, and Medicaid/Medicare contracts and benefit systems.
  • Full and current COVID-19 vaccination.
  • Current DEA
  • Access to reliable transportation.

The physical ability to provide home-based primary care.
