Position Summary:

Primary Responsibilities:

• Evaluate and treat patients in accordance with established standards of care.
• Coordinate patient services, including specialty referrals, hospital and skilled nursing facility (SNF) coordination, durable medical equipment, and home health care, as needed.
• Actively participate in daily huddles, serving as a key medical expert within the care team.
• Collaborate with the Lead Physician and Center Administrator to foster a culture of cooperation and professionalism.
• Complete all medical record documentation daily, working alongside a quality-focused coder to ensure comprehensive documentation.
• Adhere to policies and protocols set forth by Clinical Leadership.
• Regularly meet with the Lead Physician to discuss quality of care, review outcome data, and address medical record issues.
• Focus on direct patient care while balancing patient-facing time with administrative responsibilities related to patient care.

Required Qualifications:

• Graduate of an accredited MD or DO program.
• Current and unrestricted medical license or willingness to obtain a license in the state of practice, with eligibility to acquire licenses in other states as needed.
• Board Certification or eligibility for certification (ABMS or AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine preferred.
• Active and unrestricted DEA license.
• Medicare and Medicaid Provider Numbers.
• Exceptional interpersonal and communication skills with colleagues and patients.
• Commitment to team-based care.
• Willingness to learn and adapt to a value-based care model.
• Strong focus on patient/customer service.
• Basic computer proficiency, including email and electronic medical records (EMR).

Preferred Qualifications:

• Familiarity with HEDIS/STARS metrics.
• Knowledge of CAHPS and HOS.
• Experience in medically underserved and culturally diverse communities.
• Proficiency with electronic health records (EHR).
• Background in Medicare Advantage Global Risk Models.
