Position Summary:
Join our dynamic care team at Sage Health, where collaboration among Physicians, Advanced Practice Professionals, Nurses, Social Workers, Case Managers, Care Coordinators, Medical Assistants, Behavioral Health Support, quality-focused Coders, Referral Coordinators, and more is at the heart of what we do. Primary Responsibilities:
- Evaluate and treat patients in accordance with established standards of care.
- Coordinate patient services, including specialty referrals, hospital and skilled nursing facility (SNF) coordination, durable medical equipment, and home health care, as needed.
- Actively participate in daily huddles, serving as a key medical expert within the care team.
- Collaborate with the Lead Physician and Center Administrator to foster a culture of cooperation and professionalism.
- Complete all medical record documentation daily, working alongside a quality-focused coder to ensure comprehensive documentation.
- Adhere to policies and protocols set forth by Clinical Leadership.
- Regularly meet with the Lead Physician to discuss quality of care, review outcome data, and address medical record issues.
- Focus on direct patient care while balancing patient-facing time with administrative responsibilities related to patient care.
Required Qualifications:
- Graduate of an accredited MD or DO program.
- Current and unrestricted medical license or willingness to obtain a license in the state of practice, with eligibility to acquire licenses in other states as needed.
- Board Certification or eligibility for certification (ABMS or AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine preferred.
- Active and unrestricted DEA license.
- Medicare and Medicaid Provider Numbers.
- Exceptional interpersonal and communication skills with colleagues and patients.
- Commitment to team-based care.
- Willingness to learn and adapt to a value-based care model.
- Strong focus on patient/customer service.
- Basic computer proficiency, including email and electronic medical records (EMR).
Preferred Qualifications:
- Familiarity with HEDIS/STARS metrics.
- Knowledge of CAHPS and HOS.
- Experience in medically underserved and culturally diverse communities.
- Proficiency with electronic health records (EHR).
- Background in Medicare Advantage Global Risk Models.
