Position Summary:

As a Primary Care Physician, you will be an integral part of a collaborative care team focused on optimizing patient outcomes. This role enables you to provide personalized care while working in a value-based, team-centered model that prioritizes comprehensive patient support.

Primary Responsibilities:

  • Provide patient-centered care according to established standards of practice.
  • Collaborate on patient care needs, including specialty referrals, hospital coordination, SNF support, durable medical equipment, and home health care services.
  • Actively engage with the care team during daily huddles, offering your medical expertise.
  • Promote a cooperative work environment, assisting the Lead Physician and Center Administrator by displaying professionalism and approachability.
  • Document patient care daily, working with a quality-based coder to ensure accurate and thorough medical records.
  • Adhere to clinical policies and protocols as outlined by Clinical Leadership.
  • Meet with the Lead Physician regularly to review patient care quality, outcomes, and documentation.
  • Focus on direct patient care, including patient-facing and related administrative responsibilities.

Required Qualifications:

  • MD or DO degree from an accredited institution.
  • Current, unrestricted medical license or ability to obtain one in Alabama, with willingness to obtain additional licenses as needed.
  • Board Certified or Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine (ABMS or AOA).
  • Active DEA license.
  • Medicare and Medicaid provider numbers.
  • Strong interpersonal skills for effective collaboration with patients and colleagues.
  • Committed to the team-based care model and adaptable to a value-based care approach.
  • Strong commitment to patient service and quality of care.
  • Basic proficiency with computer systems, including email and EMR.

Preferred Qualifications:

  • Knowledge of HEDIS/STARS metrics.
  • Familiarity with CAHPS and HOS measures.
  • Experience with medically underserved and culturally diverse populations.
  • Proficiency in electronic health records.
  • Experience in Medicare Advantage and Global Risk models.
