Position Overview:
Join our dedicated team committed to delivering quality, patient-centered care in Pinebrook/Mobile, AL. Our collaborative care team includes Physicians, Advanced Practice Professionals, Nurses, Social Workers, Case Managers, Care Coordinators, Behavioral Health Support, Coders, Referral Coordinators, Medical Assistants, and more. This integrated support system allows our primary care physicians to focus on quality over quantity, ensuring more time with each patient for a meaningful, personalized healthcare experience.
Key Responsibilities:
- Provide high-quality, patient-centered care, evaluating and treating patients according to established standards
- Coordinate patient services as needed, including specialty referrals, hospital transitions, durable medical equipment, and home health care
- Actively participate in daily care team huddles, sharing medical insights and collaborating on patient care strategies
- Work alongside the Lead Physician and Center Administrator to foster a cooperative, professional practice environment
- Complete daily medical record documentation, collaborating with quality-based coders to optimize documentation accuracy
- Adhere to clinical policies and protocols as outlined by Clinical Leadership
- Engage in regular quality reviews with the Lead Physician, discussing care outcomes and medical record standards
- Maintain a clinical focus on direct patient care, balancing patient-facing time with related administrative duties
Qualifications:
- MD or DO degree from an accredited medical program
- Current, unrestricted medical license in Alabama (or willingness to obtain), with eligibility to practice across the region as required
- Board Certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine (ABMS or AOA)
- Active and unrestricted DEA license
- Medicare and Medicaid Provider Numbers
- Strong interpersonal and communication skills with both colleagues and patients
- Commitment to a team-based care model and adaptability to a value-based care setting
- High standards of patient/customer service
- Basic computer skills, including proficiency with email and electronic medical records
Preferred Qualifications:
- Familiarity with HEDIS/STARS and knowledge of CAHPS and HOS
- Experience in medically underserved and/or culturally diverse communities
- Prior use of electronic health records
- Experience with Medicare Advantage Global Risk Models
