Highlights
Community Physician Network is seeking a Physician to join our outpatient practice at our Family Medicine East Health Pavilion on the east side of Indianapolis, IN.
•    Well-established and outpatient
•    Shared call schedule with 24/7 RN triage
•    East Health Pavilion also houses Family Medicine, Pediatrics, Dermatology, full lab services, X-ray and PT and rehab
•    Patient Centered Medical Home (PCMH) practice model with pharmacist, dietician and care navigation nurse
•    Electronic Medical Record system (Epic)

Licensure & Certifications
•    Board certified/board eligible in Family Medicine
•    Licensed in Indiana or eligible for medical licensure in Indiana
