Monogram Health treats patients with chronic diseases including CKD, and ESRD.  Monogram MDs are clinical leaders who contribute to the development and oversight of clinical strategies, policies, programs, processes, guidelines, and operations that drive improved patient health outcomes for chronically ill patients. The MD oversees the delivery of care management services, ensuring that patient care is high quality, consistent and compliant. 

Monogram Health has a full-time telehealth opportunity for Nephrology candidates living in Nashville, Tennessee. Monogram actively manages patients with a wide range of acute and chronic conditions.  Everything we do is to design access to health care and health outcomes for chronic kidney patients, by supporting their individual needs in their own homes.

Roles and Responsibilities:

  • Remote (Work from home), Monday-Friday 8:30 am-5:30 pm
  • Serve as a collaborator, partner/mentor in the training of a small POD of NPs and other providers to provide high-quality care
  • Apply evidence-based criteria in determining the appropriateness and necessity of care; in particular, in determining whether dialytic or non-dialytic care is the correct evidence-based pathway
  • Reduce unnecessary or preventable episodes of care, including hospital readmissions
  • Identify and make practical, pointed interventions to improve member care
  • Review Annual Wellness Visit assessments to ensure proper coding and provision of medical diagnoses across multiple comorbid conditions
  • Conduct meaningful end-of-life and treatment options discussions with patients
  • Utilize data tools to report, monitor, and drive value-based care
  • Conduct retrospective reviews of hospitalizations and dialysis treatment episodes to ensure Monogram's services are effectively rendered
  • Oversee the continual refinement of clinical strategies, evidence-based clinical guidelines, and care management programs for effectiveness and efficiency within the Nashville market
  • Prepare clinical care reports in a timely fashion
  • Analyze data to build member care plans and ensure care goals are achieved
  • Achieve clinical goals and metrics to deliver improved health outcomes at a lower total cost of care

Position Requirements:

  • Must live in Nashville and be licensed in the state of Tennessee
  • Must be Board-certified in Nephrology
  • 2+ years of Nephrology experience 
  • Desire to practice in a forward-looking environment focused on population health management
  • Demonstrated experience applying evidence-based clinical criteria
  • Experience in renal care, chronic illness, geriatrics, and/or hospitalist medicine
  • Comfort and skill in end-of-life discussions and advanced care planning
  • Must have experience with Medicaid, Medicare, quality improvement, medical utilization management, and risk adjustment
  • Experience integrating acute and behavioral health care and appreciation of social determinants of health and the value of social workers
  • Must be willing to do in-home visits with NP 1 to 2 times a month / if needed
  • Expertise in managing medication panels for complex patients
