Primary Care Physician
- Provide comprehensive primary care to seniors in their homes with a Medical Assistant partner.
- Manage a smaller patient panel (8-10 per day) for personalized care.
- Spend up to 45 minutes per visit to build strong patient relationships.
- Work Monday-Friday, no weekends.
Physician Leader
- Clinical oversight of Advance Practice Providers
- Oversee clinical operations, mentor providers, and ensure quality standards.
- Maintain a reduced patient panel while focusing on leadership duties.
- Monday-Friday schedule with no weekends.

Requirements
- New Jersey Medical License (or ability to obtain)
- Experience with geriatric chronic care management
- Board Eligible or Board Certified
- Successful completion of residency program required.
