Care Connections is a short-term, multidisciplinary program dedicated to serving patients with complex medical and social needs. Based in Lancaster, PA, our integrated care model includes inpatient, outpatient, and home-based visits, allowing for comprehensive and deeply personal care.
We’re seeking a board-certified or board-eligible physician in Family Medicine, Internal Medicine, or Geriatrics who is passionate about serving an underserved population and thrives in a team-based environment.
- Enjoy a Monday–Friday dayshift schedule with minimal shared call
- Collaborate with a full team that includes:
- Physicians and Advanced Practice Providers
- Nurse Case Manager, Patient Care Navigator
- Clinical Pharmacist, Social Workers, Clinical Service Specialists
- In-house Chaplain
- Focus on whole-person care and provider satisfaction in a flexible care setting
Key Responsibilities
Qualified individuals must be able (with or without reasonable accommodation) to:
- Evaluate patients and determine appropriate care plans
- Order and interpret diagnostic tests
- Monitor treatment efficacy, lab results, and medication adherence
- Administer/prescribe treatment, therapies, and vaccinations
- Explain test results and procedures to patients and families
- Coordinate care with interdisciplinary health team members
- Refer patients to specialists when necessary
Minimum Qualifications
- MD or DO from an accredited medical school
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics
- Completion of an accredited Family Medicine or Internal Medicine Residency
