Primary Care Physician

• Provide comprehensive primary care to seniors in their homes.
• Manage a smaller patient panel (8-10 per day) for personalized care.
• Spend up to 60 minutes per visit to build strong patient relationships.
• Work Monday-Friday, no weekends.

Physician Leader

• Lead a team of healthcare professionals in providing high-quality senior care.
• Oversee clinical operations, mentor providers, and ensure quality standards.
• Maintain a reduced patient panel while focusing on leadership duties.
• Monday-Friday schedule with no weekends.

Requirements:

• Indiana Medical License (or ability to obtain)
