Position Details:

• Full-Time opportunity in Orange County, CA
• Location: Irvine, Mission Viejo, Aliso Viejo (in home visits and SNIFF)
• Covering Assisted Living Facilities (80%), Covering Home Hospice (20%)
• Hours are Monday - Friday 8:00 am - 5:00pm
• Energetic, self-starter who can work independently and contribute to team environment
• Occasionally cover weekends & extended patient care hours as needed for Home Health
• Feel comfortable in a range of settings
• Flexibility in shifts to develop and manage patient panel of geriatric palliative care
• Participate in community service and philanthropic activities

Requirements:

• M.D. or D.O. Degree
• Board Certified in Family Medicine or Internal Medicine (ABFM or ABIM)
• Fellowship in Hospice and Palliative Care Preferred
• EMR proficient (we use EPIC)
• Multilingual skills preferred
• Ability to work independently and establish strong rapport with geriatric patients
