Physician Leader - Primary Care

At HarmonyCares, we prioritize compassionate, hands-on care that directly enhances the quality of life for our elderly patients in the comfort of their own homes. We are currently seeking a physician to lead a dedicated team of Jacksonville area providers practicing medicine in a way that fosters meaningful connections.

• 20% - 40% Collaboration time, dedicated to coaching, mentoring, and guiding the providers you lead.
• Smaller patient panels seeing only 6-10 patients per day, enabling our providers to spend 30-60 minutes with each patient.
• Collaborative Team environment.
• Our providers are driven to their visits in a company vehicle, by a Clinical Partner who provides clinical support during visits.
• M-F 8:30-5, plus some telephonic (back-up) call
