Kent Hospital, part of the Care New England Health System in Rhode Island, is seeking a physician to join our innovative and successful Hospital at Home (HaH) team. Kent’s HaH program, which had its start in 2022, has served over 1000 patients, delivering high quality hospital-level care to patients in their homes. This care delivery model is part of a growing national trend and has been shown to improve patient outcomes, reduce complications, lower hospital readmission rates, and enhance patient satisfaction - all while easing the strain on hospital capacity.
Kent HaH patients receive two in person nursing visits daily, one physician visit daily (typically in-person, but occasionally virtual), are monitored 24/7 via telemetry via a light-weight biopatch sensor, and have access to a range of inpatient services such as labwork, mobile diagnostic imaging, PT/OT, and physician consults.
This position is well suited to a physician who excels in patient-centered care, builds strong relationships with patients and families, enjoys working in an innovative practice environment, and thrives in a collaborative and multidisciplinary team.
