CHCW is an integrated community health organization dedicated to providing accessible and affordable healthcare to residents of Yakima and Kittitas Counties. Since its founding in 1993 by the Central Washington Family Medicine Residency program, CHCW has expanded to deliver comprehensive services, including full-spectrum primary care, pediatrics, behavioral health, addiction treatment, dental care, and senior residential care.

Position Highlights

• Provide primary care services to senior care clients across contracted skilled nursing facilities (SNFs), adult family homes (AFHs), and assisted living facilities (ALFs).
• Serve as the Medical Director for individual SNFs.
• Collaborate with the SR Care Clinical Director and Program Manager to deliver integrated care tailored to patient and facility needs.
• Manage inpatient admitting, clinical care, and administrative responsibilities.
• Engage in geriatric-focused clinical care with a preference for internal medicine or geriatric medicine background.
• Bilingual Spanish skills are a plus.

Practice Details

• Patient Volume: Average of 14-17 patients/day, typically at one facility/day.
• EMR: GEHRIMED.
• Location: Central Washington, known for its stunning scenery, outdoor activities, and vibrant local culture.

Compensation and Benefits

• Base Salary: $220k–$230k DOE, with additional fellowship and productivity incentives.
• Oversite compensation: An additional $12,000-$16,000 per year.

Qualifications

• MD/DO or IMG equivalent.
• Completion of a US-accredited FM or IM Residency.
• Preference for Geriatric Medicine certification or FM/IM with geriatric qualifications (AMDA certification or training a plus).
• Licensed or eligible for licensure in Washington.
• DEA certification and board eligibility/certification required.
• Strong professional references and credentialing approvals.
