The Physician must hold a Doctor of Medicine Degree (M.D) or a Doctorate of Osteopathic Medicine (D.O), have completed residency and be Board Eligible or Board Certified with a current State of TN Medical License, DEA certificate, malpractice insurability and authority to provide medical services on behalf of all governmental and private insurers, health maintenance organizations, preferred provider organizations, self-insured employers, hospitals and any other organizations with which the group contracts to provide health care services.

We support our patients by providing blood pressure monitoring programs, case management and care coordination, chronic care management, diabetic programs, home visits, pharmacists on-site, social workers, and behavioral therapists.
