Job Description

This is a unique position aimed at providing neurologic specialized healthcare to individuals and families at risk for or suffering from neurologic disease or illness. Our Neurology department is supported by a team of highly skilled and expert physician and advance practice provider (APP) neurologists looking to expand their team. The nurse practitioner or physician assistant will be responsible for assessing and managing the healthcare needs and problems within neurology, emphasizing health promotion and disease prevention as well as the diagnosis and treatment of acute and chronic illness. The APP will function as part of the provider team to provide ambulatory and telehealth care to the department’s panel of patients and may also have a defined panel of patients who identify the APRN or PA as their neurologist. Under the supervision of the Neurology Chair, Lead APP, Medical Director and Ambulatory Clinic Section Chief, the APP will coordinate services throughout the Neurology Department to drive the highest quality neurological care. Primary clinic location is at the main campus, academic medical center building. Key Clinical Responsibilities:
  • Assesses physical and psychosocial health status of patient and family by taking health histories and conducting comprehensive physical examinations.
  • Orders appropriate diagnostic/screening tests to evaluate patient’s current health status.
  • Formulates diagnosis and implement treatment plan to manage acute illness and stable chronic illness consulting with and making a referral to physician colleagues and other health care providers as indicated.
  • Consults and collaborates with a department physician, APP or appropriate specialist to plan and manage the care for patients with unstable or complex acute or chronic illness. Facilitates transfer of patient to the primary care physician when the complexity of care exceeds scope of practice.
  • Evaluates patient’s progress during follow up visits and implement revised plan of care.
  • Prescribes and/or dispenses medication.
  • Provides patient education with emphasis on the promotion of health and wellness and the prevention of illness and disability.
  • Provides guidance and counseling for both individuals and families.
  • Demonstrates patient advocacy that insures the patient’s right to privacy, confidentiality, and informed consent.
  • Serves as a mentor for new APPs and students.
  • Assumes responsibility to renew APRN or PA license in a timely manner and maintain DEA registration.
  • Attends continuing education programs to ensure ongoing professional growth and development to meet licensure requirements.
  • Performs other duties as required or assigned.
Requirements:
  • Graduate of an accredited APRN or PA academic program
  • National specialty certification as FNP, WHNP, PNP, ANP or certification through the National Commission on Certification of Physician Assistants (NCCPA)
  • Eligible for RN licensure in the State of NH (APRN only)
  • Eligible for APRN or PA licensure in the State of NH
  • Eligible for Federal DEA Registration
  • BLS certification
