Position Summary:

Join a supportive, collaborative team of healthcare professionals dedicated to providing high-quality care to our community. Our care team includes Physicians, Advanced Practice Professionals, Nurses, Social Workers, Case Managers, Care Coordinators, Medical Assistants, Behavioral Health Support, Quality-Based Coders, Referral Coordinators, and more. This extensive support network allows our primary care physicians to focus more on patient care with fewer daily visits, leading to a meaningful impact on patient outcomes.

Primary Responsibilities:

  • Provide patient-centered care in an outpatient setting, adhering to established standards of care.
  • Coordinate patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care as needed.
  • Actively participate in daily huddles with the care team, sharing medical insights and collaborating on patient management.
  • Support the Lead Physician and Center Administrator by fostering a collaborative and professional team atmosphere.
  • Complete daily medical documentation with support from quality-based coders to ensure thorough, accurate records.
  • Adhere to policies and protocols set by Clinical Leadership.
  • Regularly review care quality, outcomes, and medical records with the Lead Physician.
  • Balance clinical responsibilities, including patient-facing time and administrative duties, all focused on direct patient care.

Required Qualifications:

  • MD or DO degree from an accredited medical program.
  • Current, unrestricted medical license or ability to obtain a license in the practicing state; willingness to gain licenses in additional states as needed.
  • Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine (ABMS or AOA).
  • Active, unrestricted DEA license.
  • Medicare Provider Number.
  • Medicaid Provider Number.
  • Strong interpersonal and communication skills with colleagues and patients.
  • Commitment to team-based care.
  • Adaptability and commitment to value-based care principles.
  • Excellent patient service skills.
  • Basic computer skills, including proficiency with email and EMR systems.

Preferred Qualifications:

  • Familiarity with HEDIS/STARS measures.
  • Understanding of CAHPS and HOS.
  • Experience serving medically underserved or culturally diverse populations.
  • Proficiency with electronic health records.
  • Experience in Medicare Advantage Global Risk Models.
