Join Our Primary Care Team ! 

 

VA Illiana Health Care System is currently seeking outstanding full-time Family Practice or Internal Medicine trained physicians to join our Community Based Primary Care Clinic team in our Peoria Clinic. This incumbent will be responsible to provide comprehensive care, under the Patient Aligned Care Team (PACT) model, to a panel of patients managed under PCMM guidelines. The incumbent is expected to be attentive to patient education, preventive care and be able to work well in an interdisciplinary team to provide high quality primary care. The incumbent will be responsible for providing comprehensive medical care by performing duties to include, but not limited to the below responsibilities:

  • Manages chronic conditions, evaluates and treats inter-current medical conditions
  • Collaborates with a Patient Aligned Care Team (PACT) to provide accessible, comprehensive, coordinated, continuous care to an assigned panel of patients
  • Performs a variety of disease prevention related activities including gender and age specific screenings, mental health screenings, and health promotion activities.
  • Screens for, evaluates and treats uncomplicated common mental health disorders, including depression and common addictions, with referrals made to mental health providers for further evaluation and treatment as appropriate
  • Primary care physicians will have responsibility for supervision of or collaboration with primary care mid-level providers
  • Physicians are expected to provide clinical oversight and to participate in peer review of other primary care providers through periodic chart reviews
  • Primary care physicians will act as surrogates for absent primary care providers when needed, to include review of CPRS view alerts
  • Primary care physicians will regular attend primary care staff meetings, departmental meetings and medical staff meetings
  • Timely documentation (notes and encounter forms) is necessary in all areas in accordance with facility policy. All administrative work should be completed in a timely fashion
  • Clinical reminders should be addressed as medically appropriate. Quality of care should meet professional standards at all times

Work Schedule: Monday-Friday; 7:45 a.m. - 4:15 p.m.

Federal Retirement Plan:

  • VA employees are covered by the Federal Employees Retirement System (FERS). FERS is a three-tiered retirement plan composed of 1) Social Security benefits, 2) FERS-RAE basic benefits (pension), and 3) Thrift Savings Plan (TSP) which is a 401K-type plan
  • How is the pension funded? VA employees automatically contribute 4.4% of their salary each pay period and the VA contributes 12.2% of the employee’s salary each pay period for a total annual contribution percentage equal to 16.6% of the employee’s salary
  • Similar to the 401(k) savings plans widely available in the private sector, the TSP allows employees to tax defer a portion of their income each year, subject to the requirements of the Internal Revenue Service. For calendar year 2020, employees under the age of 50 may contribute up to $19,500, while those over age 50 are eligible to make catch-up contributions that allow them to defer up to $25,500.
  • The Federal Government also provides an automatic basic contribution (1% of salary) and up to 4% of salary in matching contributions, depending on the amount of the employee contribution
  • For retired military personnel, these benefits are in addition to full monthly retirement pay or pension
  • In addition to FERS, VA provides health care insurance coverage for retired personnel
