Position Summary:
Join our dedicated care team in providing quality-focused outpatient primary care in Gulfport, MS. Our multidisciplinary team includes Physicians, Advanced Practice Professionals, Nurses, Social Workers, Case Managers, Care Coordinators, Medical Assistants, Behavioral Health Support, quality-focused Coders, and Referral Coordinators. This integrated support structure enables our physicians to spend more time with each patient, fostering a high standard of care in a collaborative environment.
Primary Responsibilities:
- Provide patient evaluations and treatment in accordance with best-practice standards.
- Coordinate patient services, such as specialty referrals, hospital/SNF coordination, durable medical equipment, and home health care when needed.
- Participate as a key medical resource in daily care team huddles, promoting teamwork and collaboration.
- Partner with the Lead Physician and Center Administrator to foster a cooperative, professional environment.
- Complete medical documentation daily, collaborating with quality-based coders to ensure thorough and accurate patient records.
- Adhere to clinical protocols and guidelines as defined by Clinical Leadership.
- Collaborate with the Lead Physician on quality-of-care metrics, outcome reviews, and medical record standards.
- Dedicate time to direct patient care, including patient-facing interactions and related administrative tasks.
Required Qualifications:
- MD or DO degree from an accredited medical school.
- Current and unrestricted medical license in MS (or eligibility to obtain) with flexibility for licensure in additional states, if needed.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine (ABMS or AOA).
- Active, unrestricted DEA license.
- Medicare and Medicaid Provider Numbers.
- Strong interpersonal and communication skills, with a commitment to patient-centered care.
- Enthusiastic engagement in team-based care.
- Willingness to adapt to a value-based care model.
- Excellent customer service skills.
- Basic computer proficiency, including EMR systems.
Preferred Qualifications:
- Knowledge of HEDIS/STARS, CAHPS, and HOS.
- Experience in underserved or culturally diverse settings.
- Familiarity with Medicare Advantage Global Risk Models.
- Experience working with electronic health records.
