*Guaranteed Hours: 40hrs per week*

*Description:*
* Seeking an RN care coordinator for primary care clinic in Alaska.
* This position will coordinate care for patients in several remote locations for chronic care appointments, follow up for recent hospitalization, and an advocate to patient needs.
* Strong organizational skills, communication skills to care providers and patients, proficient in computer, telephone communication a must.
* Experience in Care Coordination/Case Manager required.
* Housing will be provided probable with roommates, so cannot have pets, or S/O.
* Current COVID and flu vaccine required.
* Alaska is not a compact state, applicant must have a current and unencumbered license in home state with an Alaska license application.
