POSITION SUMMARY
The Crisis Therapist is responsible for completing assessments, determining safety, danger of DTO/DTS, and the level of care needed by clients presenting in the crisis department. They welcome and coordinate the determination of care for all clients in coordination with the medical provider, the paramedic, or RN.

ESSENTIAL RESPONSIBILITIES

The crisis therapist initiates an assessment (or reviews accompanying assessment if completed by participating provider) within 30 minutes of the client's arrival to determine if the individual meets basic requirements for admission to a Polara Health inpatient facility. Including:
Crisis Screening and assessment
Columbia Suicide Screening Scale
Broset Violence Checklist (if indicated)
Determine the appropriate level of care using the LOCUS evaluation
Completing Safety Plan and coordinating appropriate care provisions
Coordinates sending out packets to find appropriate placement for clients not admitted or diverted
Collaborates with client supports, Nursing Staff and Provider to develop and implement an individualized person-centered plan of care to ensure safety and appropriate therapy interventions
Gathers information from the patient and available parties to provide a thorough assessment of what the patient's immediate needs are for treatment and safety, and appropriately documents them
Evaluates needs and safety concerns of the client
Collaborates with the interdisciplinary treatment team to assess packets, and determine the appropriateness of referrals for admission.
Demonstrates effective engagement skills; inspires hope and promotes recovery and resiliency providing education and community resources based on the intervention provided
Demonstrates effective documentation skills, ensures all interactions are documented in the designated electronic medical record in accordance with policies, procedures, and industry and/or organizational standards of practice.
Obtains signatures from patients regarding treatment as necessary
Models appropriate physical and emotional boundaries with clients and co-workers.
Provides emotional support to help clients manage negative symptoms and utilize coping strategies to enhance their quality of life and daily functioning
Maintains a working knowledge of the Title 36 Involuntary Commitment process completing Emergency admission and pre-petition screening forms completely and thoroughly as necessary.
Intervenes pro-actively with clients in situations where clients may be escalating emotionally and/or behaviorally using approved non-violent crisis intervention techniques, including the least restrictive interventions appropriate for the situation
Attends and participates in shift reports, site meetings, and trainings as mandated by agency policy and procedure
Communicates effectively and promotes quality service within a compassionate environment by treating clients and co-workers with respect and dignity
Helps to maintain a clean and orderly environment in all client, staff and facility areas
Supports a work climate that emphasizes collective team performance and demonstrates Polara Health's core competencies and values.

QUALIFICATIONS
21 years of age or older; or minimum 18 years old and licensed or certified under A.R.S. Title 32 and providing services within the personnel member's scope of practice; or minimum 18 years old for select administrative positions
Must be able to provide or obtain an AZDPS Level One Fingerprint Clearance Card
Must possess and maintain a valid Driver License, current automobile insurance, and a driving record that meets the guidelines and requirements of the organization
CPI and CPR/First Aid certification required (training provided by Polara Health)
Master's degree in Counseling, Psychology, Social Work, or a related field.
Current licensure or eligibility for licensure as a Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or similar license in the state of Arizona.
Strong ability to maintain strict confidentiality and handle highly confidential information with professionalism
Must be able to work independently as well as collaboratively within a team setting
Must have strong interpersonal communication, problem solving, and teambuilding skills
Must be detail oriented, have excellent organizational skills and be flexible
Must demonstrate clinical competencies essential to effective job performance
Actively participates in performance management and coaching
Demonstrates respect for consumer confidentiality practices
Technical, industry specific knowledge about healthcare, behavioral health, crisis services, and clinical standards of practice; this includes knowledge of Medicaid managed care, behavioral health assessment and recovery concepts.
Ability to work well under pressure in a fast-paced, time sensitive environment with shifting priorities and multiple deadlines
Show appreciation for and ability to respond to cultural differences, and adjust communication practices accordingly in order to minimize cross-cultural misunderstandings
Consistent work attendance and compliance with scheduled work hours (excludes approved FMLA absences)
Responsible for complying with Polara Health's Corporate Compliance Program as it applies to individual job duties, the department, and the company
Must comply with all Polara Health policies and procedures

PHYSICAL DEMANDS & WORKING CONDITIONS
The nature of work may generate considerable physical (transporting, moving, assisting clients, etc.), mental and visual demands.
