*JOB DUTIES*
* Conduct initial and periodic assessments of home health pediatric patients to evaluate rehabilitative and functional status.
* Re-evaluate the patient's need for occupational therapy and assess the outcomes of care on a regular basis, adjusting the plan of care as necessary.
* Develop and revise the plan of care based on evaluation results to ensure quality and continuity of care for the patient.
* Identify occupational deficits such as challenges with strength, fine motor coordination, sensory dysfunction, motor planning, self-care (including feeding), and recommend potential interventions.
* Evaluate the home environment to determine whether modifications or recommendations are needed to enhance the patient's functional ability.
* Assess the need for durable medical equipment (DME) and other assistive devices, providing recommendations as appropriate.
* Follow infection control procedures in accordance with OSHA guidelines to ensure the safety of both staff and patients.
* Coordinate services and inform the physician and other healthcare team members of any changes in the patient's condition or needs.
* Supervise, guide, and provide clinical direction to other occupational therapy personnel as required or requested, ensuring quality care and adherence to professional standards.
* Participate in professional development by engaging in in-service programs and continuing education to maintain or improve clinical competency.
* Occupational Therapist provides effective communication to patients/clients, their family members, team members, and other health care professionals.
*QUALIFICATIONS*
* Graduate of an accredited Diploma, with Bachelor's, Master's, or Doctorate degree
* One year of experience as an Occupational Therapist in a pediatric clinical care setting preferred
* Pediatric Home health experience preferred
* Reliable transportation
* Current Texas State license as an Occupational Therapist
* Current Driver's License
