RN Clinical Manager
Overview:
We are seeking a highly skilled and compassionate Registered Nurse Case Manager (BSN) to join our healthcare team in Savannah, Georgia. As a Registered Nurse Case Manager, you will play a crucial role in coordinating and managing the care of patients, ensuring they receive the highest quality of healthcare services. This position requires expertise in acute care, ER and pediatrics, as well as strong knowledge of physiology and medical terminology. The ideal candidate will have experience in managed care and utilization management, with a focus on patient assessment and medical records management. Knowledge of HIPAA regulations is essential to maintain patient confidentiality.
Responsibilities:
* Collaborate with *physicians, specialists, and healthcare teams* to coordinate patient care.
* *Monitor cases* to ensure documentation is compliant with regulatory agencies and third-party payer requirements.
* Ensure patients *meet admission criteria* and oversee the decision to admit patients to services.
* Manage the *assignment of caregivers* and clinicians to patient cases.
* Provide *clinical guidance, instruction, and performance coaching* to care staff.
* Assist clinicians in *setting treatment goals and care priorities* for patients.
* Participate in *case conferences, multidisciplinary team meetings, and performance reviews*.
* Advocate for *patients' rights*, ensuring access to appropriate healthcare services.
* Work with *insurance providers* to ensure proper utilization of resources.
* Support *quality assurance (QA) and performance improvement (PI) activities*.
* Supervise and evaluate direct care staff, ensuring adherence to internal policies and industry standards.
* Participate in *staff training, development, and corrective action plans* as needed.
* Maintain relationships with *referral sources, community partners, and professional organizations*.
* Participates in sales and marketing initiatives.
* Participates in the interviewing, hiring, training, and development of direct care clinicians. Evaluates their performance relative to job goals and requirements. Coaches staff and recommends in-service education programs, when needed. Ensures adherence to internal policies and standards.
*Qualifications:*
* Graduate of an accredited School of Nursing.
* Current state license as a Registered Nurse.
* Proof of current CPR.
* Valid driver's license, auto insurance and reliable transportation.
* Two years as a Registered Nurse with at least one-year of management experience in a home care, hospice or equivalent environment, preferred.
* Home health experience, preferred.
-Strong knowledge of prior authorization packages is highly desirable
- Strong knowledge of physiology, medical terminology, and HIPAA regulations
- Experience in pediatrics is highly desirable
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Strong organizational and time management skills
- Proficient computer skills for documentation purposes
