Saint Joseph Living Center is proud to be a non-profit, 120-bed Skilled Nursing Facility located in Windham, CT. We offer short-term rehabilitation, long-term care, and end-of-life care. We are a Catholic faith-based community sponsored by the Diocese of Norwich, but we welcome residents and staff of all faiths. The Center features both indoor and outdoor amenities, including a Chapel with daily mass and a beautiful enclosed courtyard.
We are currently seeking an RN to join our incredible nursing team as a Subacute Case Manager!
Position Summary: As a member of the nursing team and reporting to the Director of Nursing, the RN Subacute Case Manager will oversee and direct the day-to-day activities of the 30-bed subacute unit.

Primary Responsibilities:
Assumes responsibility for all nursing care provided to residents within the subacute unit; knows the residents' conditions at all times; observes and supervises nursing care according to physician's orders and in conformance with recognized nursing techniques and procedures, established standards and administrative policies.
Fulfills the Case Management role for Residents who are on a Managed Care stay
Recognizes and interprets symptoms, reports on residents' conditions and assists with or institutes remedial measures of adverse developments.
Assists in maintaining a physical, social and psychological environment which is conducive to the best interest and welfare of residents.
Instructs staff in the use of policies and procedures and assures that all policies and procedures are followed by nursing staff.
Qualifications:
Current RN license in Connecticut
Supervisory experience in a SNF preferred
Managed care experience preferred
Must have excellent clinical assessment skills
Must be able to communicate effectively orally and in-writing.
Must be able to demonstrate organizational skills.
Represent the Organization both onsite and in the community
Maintain regulatory compliance in accordance with state and federal guidelines
Develop and maintain positive relationships with residents, family members, staff and medical team
Work in partnership with the Director of Nursing to ensure quality, person-centered care
Hours: Full-Time, M-F Days, On-call Coverage as needed/required.
