*Salary:* $25.50-$29.25/hour, depending on home health experience.
*Schedule:*
* Full-time, days
* Home Health clinicians will see an average of 4-7 patients per day in the patients' homes. The coverage area for these visits will typically be a portion of the overall territory covered by the care assigned where the clinician is employed. The daily or weekly coverage for the clinician can vary based on the overall patient load and number of clinicians assigned within a care center.

*Responsibilities:*
* Verify plan of care with the RN prior to each visit.
* Collaborate with physician and RN frequently to ensure patient needs are being met; Notify them as soon as possible when patient condition changes.
* Teach patients and caregivers self-care techniques.
* Check vital signs.
* Review all medications and perform a medication reconciliation at each visit.
* Perform specialized procedures ordered by physician ( i.e. foley catheter change).
* Prepare equipment and materials for treatments using the aseptic technique.
* Complete documentation timely and accurately.
*Qualifications:*
* 1+ year of clinical LPN experience is required
* Current LPN license, specific to the state(s) you are assigned to work.
* Current CPR certification.
* Valid driver's license, reliable transportation and liability insurance.
