*Hospice Care Social Worker Job Description:*
* Provide psychosocial education to patients and family caregivers about coping skills, hospice and palliative care philosophy, and nonpharmacological symptom management strategies
* Facilitate psychoeducational support groups
* Provide in-services to other service providers and organizations
* Lead community education workshops
* Plans for discharge, coordinates care, and helps clients navigate systems; identifying and linking clients with resources
* Facilitates advance care planning and lifespan planning
* Intervenes in crises
* Mediates conflicts within families, clients, and the interdisciplinary team, and between service organizations
* Participates in interdisciplinary team meetings, care planning, and ethics consultations
* Advocates on behalf of the patient and family
* Document social work activities
*Social Worker Requirements:*
Licensed Social Worker in Missouri or Illinois required.
*Social Worker Compensation*
$60,000 to 68,000 Depending on Experience
