ExecuSource has partnered with a well renown hospital system and we are assisting them on a search for an LCSW in Canton and Duluth. These are direct-hire/permanent positions and the hiring manager is actively setting up interviews!
Responsibilities:
The Palliative Care Social Worker (PCSW) provides intensive psychosocial therapeutic support services to palliative care patients and families to help meet social, emotional and financial needs related to the impact of a chronic or terminal illness, grief, and loss.
Required Qualifications:
Must be a LCSW
Advanced Certified Hospice & Palliative Social Worker ACHP-SW certification or ability to take exam within 24 months of being hired
BLS through the American Heart Association
Preferred Qualifications:
Minimum of two years working in a direct clinical therapeutic capacity
Minimum of two years working with life-threating conditions, chronic disease, palliative care and or hospice, end-of-life care, and related situations
Knowledgeable of a full range of specialized and expert clinical social work counseling services for patients families receiving palliative care
Able to provide crisis intervention, short-term and family therapy; Offers information and referral and or tangible assistance
Organizational and prioritization skills while determining severity of patient condition and prognosis
Excellent verbal and written communication skills
Basic computer skills
Schedule: Monday-Friday, 8a-5p
Pay: Ranges from $32-$40/hour based on experience
