Position Purpose: In collaboration with providers, screens patients to identify and support the delivery of the most appropriate level of care and service through the acute care episode and across the continuum to support an effective discharge plan. The acute care Case Manager works collaboratively with interdisciplinary teams both internal and external to the organization to improve patient care by assuming a leadership role in driving desired clinical and financial outcomes through effective utilization and monitoring of healthcare resources.
Hours/Shift: Full Time/Days 10 hour shifts, may occasionally work weekends and holidays

Key Responsibilities:
- Ensures the active involvement of the patient and or family in the evaluation process and provides assistance with planning and arranging post-hospital services.
- Performs discharge planning/quality of care by participating in multidisciplinary rounds to assist with the development of care plans. Serves as an advocate for patients and protects patient rights as they pertain to ethical and legal issues of confidentiality during the case management process. Ensure implementation of the plan and appropriate education is provided to ensure a safe transition of care.
- Identifies appropriate community resources for each assigned patient and works collaboratively with patients, families, multidisciplinary team members, and community agencies to achieve desired patient outcomes.
- RN Case Managers may perform utilization reviews to determine medical necessity for admission, continued stay, appropriate utilization of resources, and level of care.
- Communicates medical necessity information with the requesting Payers and whether required post discharge services are covered by the patient’s payer and informs the patient of coverage determinations.
- Participates in multidisciplinary rounds to share information and facilitate the healthcare team in meeting the patient’s medical and psychosocial needs.
- Completes review process, conducts assessments, and evaluations of admitted patients for medical, physical, psychological factors that may contribute to disease process and may require specific or complex continuing care and/or supportive services.
- Determines which cases will require medical staff review and/or UR management intervention.
- Communicates with attending physician, expected length of stay, Medicare guidelines and individual variances.
- Proficiently utilizes MCG and applies clinical guidelines and criteria appropriately to make utilization determinations.
Qualifications:
- Required Experience: Three (3) years of varied acute hospital care experience
- Education: Graduate from an accredited school of nursing or Bachelor in related behavioral/social science field
- Licensure: If RN, valid AZ RN license
- Certification: BLS from American Heart Association or American Red Cross
- Preferences: ACMA or CCM Certification is desired
Work Requirements: Able to handle multiple priorities simultaneously, use technical business office equipment, sit, stand and walk 3-5 hours per shift; Possess customer service excellence, strong oral and written communication skills. Expected duties have routine potential for exposure to blood, body fluids or tissues.
