We have six large centers and many smaller sites, many of which are strategically located in or near high-need areas. We provide high quality and affordable health care services to every member of your family. Our primary care teams are specially trained to help you manage ongoing conditions, such as asthma, heart disease or diabetes, as well as access preventive care, such as annual check-ups and immunizations.

History

NYC Health + Hospitals/Gotham Health was formed in 2015 to address the primary care needs of families and individuals in their own neighborhoods. Keeping families and individuals healthy and helping each New Yorker to live their healthiest life is the reason why we feel it is so important to provide services in as many communities as possible.'

NYC Health + Hospitals/Gotham Health neighborhood health centers empower New Yorkers to live the healthiest life possible. The dedicated teams of providers at each of our sites coordinate your care using the best clinical practices. While all of our locations provide primary care, different specialty programs are available at some of our sites to address the specific needs of that neighborhood. Please check the Gotham Health location in your community for the specialties provided at that site. Annual check-ups, immunizations, chronic disease control, and family planning are provided at most sites.

In addition, as part of the largest public health care system in the country, we provide easy referrals to a vast network of hospitals, home care services, nursing homes and specialty care providers across the city.

Affordable Health Care and Medications

Our mission is to provide health care to all New Yorkers without exception. We accept most insurance plans and offer discounted fees if you do not have insurance or if your insurance does not cover what you need. We also offer discounts on prescription medications.
