University of Chiago Medicine AdventHealth Community Palliative Care Nurse Practitioner

UChicago Medicine AdventHealth is hiring a part-time APP for their Community Palliative program.  This is a growing program which is so needed in our communities as it serves our homebound patients, giving them access to palliative care in the comfort of their homes.

• Experienced APP's preferred, but new grads welcome to apply
• Part-Time position (0.6 FTE)
• Weekly Monday - Friday schedule
• No call requirement
• Patient visits in assigned territory
• Working collaboratively with Social Workers and House Calls Physicians & APP's

EDUCATION AND EXPERIENCE REQUIRED:

• Master's degree of Nursing or Physician Assistant Studies required.

LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:

• BLS Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or job transfer date. Credentials are verified by local staff credentialing office.
• Nurse Practitioner credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date. Credentials are verified by local staff credentialing office
• Drug Enforcement Admin credentialed from the Drug Enforcement Administration.
