Highlights
Community Health Network has a unique opportunity for an experienced, family medicine trained advanced practice provider to join our Family Medicine evening clinic located in Geist (northeast Indianapolis). The clinic is designed to serve established patients managing chronic conditions by offering evening hours (5-8pm) on weekdays.
· Single, robust electronic medical record system (EPIC)

Experience
Educational background consistent with requirements for licensure and certification within the state of Indiana; 2+ years of provider experience in a primary care setting strongly preferred.
