• Schedule:  Full Time, 36 patient facing hours and 4 admin hours per week
• Call Schedule: Outpatient call rotating coverage every 3-5 weeks
• Practice Detail:  MD, DO, and APP's managing complex endocrine disorders including but not limited to; DM 1 and DM 2, pre-diabetes, gestational, insulin pump therapy, hyper/hypothyroidism, parathyroid, cushings, graves disease, adrenal insufficiency, hormone replacement, growth disorders, etc.  Alongside the physician practice you will find an AHA and MDHHS Certified Diabetes Education program staffed with RN/RD-CDCES.

• Location: We support 5 clinic locations; Kalamazoo, Allegan, Dowagiac, Portage, and Battle Creek.

Responsibilities

Ascension Borgess looking for a full time Physician Assistant or Nurse Practitioner for our Diabetes and Endocrine Department.

Position Highlights:

• 4 day work week/flexible schedule
• Provider support supervising physician Michael Valitutto, DO
• Fully supporting care team consisting of physicians, NP's, PA's, office operations assistants, MA's, RN-CDCES, RD-CDCES, RN coordinators. Joining our team gives you the opportunity to live out our mission day in and day out caring for the poor and vulnerable through our Diabetes Uninsured Clinic. Community Outreach opportunities are endless. 

Responsibilities:

• Obtains patient history and performs assessment via observation, interview and examination. Orders, performs and interprets diagnostic studies. Performs preventative health assessments, screening, immunizations and patient care.
• Provides direct treatment and management of health conditions via referral to other healthcare providers or community resources.
• Counsels and educates patients and their families/caregivers concerning preventative health, treatment options and community resources.
• Documents delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy.

Licensure/Certification/Registration:
Required Credential(s):
• Nurse Practitioner with Prescriptive Authority credentialed from the Michigan Board of Nursing obtained prior to hire date or job transfer date. Verified by local Credentialing Office.
• Registered Nurse credentialed from the Michigan Board of Nursing obtained prior to hire date or job transfer date. Verified by local Credentialing Office.
• BLS Provider credentialed from the American Heart Association (AHA) obtained prior to hire date of job transfer date. Verified by local Credentialing Office.

Education:
• Master's degree of Nursing required.
