Job Description

SSM Health Dean Medical Group, a 500+ physician multi-specialty group, is actively recruiting a full-time (1.0 FTE) Inpatient Palliative Care Nurse Practitioner to join our group at SSM Health St. Mary’s Hospital in Madison, Wisconsin.  Ideal candidate will be patient centered, service oriented and dedicated.

Opportunity 

  • 100% inpatient
  • Become part of an interdisciplinary Palliative Care program
  • Must be comfortable with having Goals of Care conversations
  • Anticipated schedule of Monday-Friday without call
  • Key responsibilities include consultations from referrals from hospitalists, patient follow-up care and symptom management
  • Variable consult schedule (1-1.5 hours per consult), with team approach to patient follow-up care
  • Average 3-4 consults per day and 4-5 follow-ups

Requirements

  • Wisconsin registered nurse license and advanced practice nurse prescriber license
  • Active and unrestricted Wisconsin DEA or the ability to obtain
  • Minimum of 1-2 years post-education experience as a Nurse Practitioner in Palliative Care or Hospice Medicine; or a Nurse Practitioner with 2-3 years’ experience as an RN in Palliative Care or Hospice Medicine
