As a Family Medicine Nurse Practitioner at West Park Family Practice, you will play a crucial role in delivering high-quality, comprehensive care to patients of all ages. Your responsibilities will include conducting thorough physical examinations, diagnosing and treating acute and chronic illnesses, and developing personalized care plans. You will also offer preventive care services, including health screenings, immunizations, and patient education to promote overall wellness.

Key Responsibilities:

• Provide patient-centered care in a family medicine setting, addressing a wide range of health concerns.
• Conduct detailed patient assessments, including history-taking and physical examinations.
• Diagnose and manage acute and chronic medical conditions.
• Develop and implement individualized treatment plans.
• Educate patients and their families about health conditions, treatment options, and lifestyle changes.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Utilize telemedicine technology for remote consultations and follow-ups.
• Maintain accurate and up-to-date medical records.

Schedule:

• Monday to Friday: 8 AM to 4 PM

Qualifications:

• Master's degree in Nursing with specialization as a Family Nurse Practitioner.
• Certification as a Family Nurse Practitioner (FNP) from a recognized certifying body.
• Valid Nurse Practitioner license in the state.
• Strong clinical skills and experience in family medicine.
• Excellent communication and interpersonal skills.
• Ability to work effectively both independently and as part of a team.
• Proficiency with electronic medical records and telemedicine platforms.
