NP for Population Health in Sunny Tucson Arizona

This is an outstanding opportunity for a dynamic FNP to be part of a new and growing Population Health program. This provider will be a member of and supported by the Population Health team. This position will allow you to care for a diverse patient population in a challenging and rewarding clinic setting. 

 

As a Nurse Practitioner in this position, you will be involved in clinical management and support of patients including Medicare Annual Wellness Exams, transition of care following hospital discharge, diabetes care and screenings, preventative medicine, ER follow ups, Same Day/Next Day appointments at our Alvernon Clinic. You will also collaborate often with medical students, residents, physicians, and other clinicians in a team approach to patient care.

 

Work Schedule:    

 

·        Full-time. M-F | 8-5 

·        In person, hybrid potential for telehealth – 20% from within clinic

·        Call Rotation (5 pm – 9 pm M-F, 1 session every 3 months)

·        Weekend moonlighting opportunity for sponsored wellness events

 

Patient Volumes:  

 

·        Cap at 16 patients per day, all appointments 30 minutes

 

Required Qualifications:

·        Experience in Primary Care is preferred

·        Solid Knowledge in Population Health

·        Current board certification

·        Current Arizona license or ability to obtain at time of application

·        Current CPR, BLS, and ACLS

·        Current prescribing privileges including a DEA license for controlled substance

·        Highly qualified new Nurse Practitioner graduates will be considered
