The PeaceHealth is seeking afull time adult gerontology nurse practitioner (AGNP), adult nurse practitioner (ANP), gerontological nurse practitioner (GNP) or family nurse practitioner (FNP) with an interest in geriatric medicineto work as a team member of the Senior Health & Wellness Center (SHWC). Nurse practitioners, working with full practice authority, have the option of practicing within an outpatient clinic environment, managing a dedicate panel or patients. Nurse practitioners with a passion for improving and/or maintaining the quality of life for the older adult are welcome to join our skilled, collaborative, and compassionate team of geriatric specialists as a provider.
Within the Senior Health & Wellness Center, anoutpatient geriatrics clinicwhich provides comprehensive primary and consultative care for adults age 65 and older, nurse practitioners serve as primary care providers and consultants to meet the needs of an expanding older adult population. In addition to comprehensive primary care, a multidisciplinary team at SHWC, operating collaboratively, also offers care management, counseling, diabetes education, foot care, geriatric specialty consultations, memory and cognitive evaluations, and medication education/management for chronic diseases.
