Highlights
This Nurse Practitioner will join our growing team of providers in the Community ProCare at Home Program practicing in Indianapolis’ and the surrounding counties. This position will provide in-home house call services to patients who have previously been hospitalized, recently discharged from a skilled nursing facility, or are homebound. Ideal candidates will have demonstrated expertise in the advanced practice of nursing with chronically ill patients and have an understanding of the specific needs of assessing patients as they navigate their health challenges. Ideal candidates will have knowledge of local resources and have prior house call or home care experience. Additionally, ideal candidates will have the ability and desire to work independently and have genuine interest in delivering care in the home setting.
Licensure & Certification
Educational background consistent with requirements for licensure and certification within the state of Indiana.
