SSM Health St. Mary's Hospital in St. Louis, Missouri, is seeking a dedicated and skilled Advanced Practice Provider (APP) to join our Primary Care Clinic team. The APP will provide comprehensive, patient-centered care, including assessment, diagnosis, treatment, and follow-up for a diverse patient population. This role involves collaborating with physicians, nurses, and other healthcare professionals to deliver high-quality care and improve patient outcomes.
Key Responsibilities:
- Perform comprehensive health assessments, including patient histories, physical examinations, and diagnostic tests.
- Diagnose and manage acute and chronic illnesses, injuries, and conditions.
- Develop and implement individualized treatment plans in collaboration with patients and their families.
- Prescribe medications and other treatments in accordance with state and federal regulations.
- Educate patients and their families on health maintenance, disease prevention, and management of chronic conditions.
- Perform routine procedures such as suturing, wound care, and immunizations.
- Collaborate with physicians, specialists, and other healthcare professionals to ensure comprehensive patient care.
- Document patient care activities accurately and timely in electronic health records (EHR).
- Participate in quality improvement initiatives and contribute to the development of clinical protocols and guidelines.
- Stay current with medical advancements, evidence-based practices, and state and federal healthcare regulations.
Qualifications:
- Master's or Doctoral degree in Nursing or Physician Assistant Studies from an accredited institution.
- Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Missouri.
- Certification as a Nurse Practitioner by a recognized certifying body (e.g., AANP, ANCC) or as a Physician Assistant (NCCPA).
- Minimum of 2 years of experience in primary care, family practice, or internal medicine preferred.
- Strong clinical skills and the ability to work independently and as part of a multidisciplinary team.
- Excellent communication, interpersonal, and organizational skills.
- Commitment to providing compassionate, patient-centered care.
- Proficiency with electronic health records (EHR) systems.
